Community Integration Specialist

JEVS Human Services is opening a new program this Fall and we are currently seeking detail-oriented, resourceful and compassionate individuals to join the team as a Community Integration Specialist. In this role, you will be responsible for helping individuals transition between services and adapt to life changes.

Job Duties:

  • Support the individuals connection with friends and their inclusion in the community of their choice.
  • Teach skills to help individuals use community resources and interact socially with others.
  • Build collaborative, professional relationships with the individual and others on the support team.
  • Work in partnership with the individual to track progress toward goals and adjust services as needed and desired by individual.
  • Monitor the individual’s physical and emotional health
  • Identify risk and behaviors that can lead to a crisis and use effective strategies to prevent or intervene in the crisis in collaboration with others.
  • Take the lead role with a network of current individuals and work with new and developing relationships to increase the volunteer opportunities and social activities in the community.

Skills & Requirements

  • Bachelor's degree or two years of college course work in psychology, education, social work, or other appropriate field required; appropriate experience may be considered in lieu of Bachelor's degree
  • 2+ years' experience in a related field required
